EBウイルス関連血球貪食性リンパ組織球症（EBV-HLH）は、EBVのCD8+ T細胞への異所性感染とそのモノクローナルな増殖により引き起こされる。我々は、EBV感染CD8陽性T細胞が特定のTCR Vβを有しCD5発現の低下した異常活性化細胞として同定可能であることを報告し、さらにサイトカインプロファイルの特徴を明らかにした。ただし活性化CD8陽性T細胞におけるCD5発現の低下自体はEBV-HLHに特異的ではないため、TCR Vβレパトア解析を併用し評価する必要がある。本研究により、これらの細胞解析とサイトカイン解析の併用はEBV-HLHの診断、病態把握に有用であることが明らかにされた。
Epstein-Barr virus (EBV)-associated hemophagocytic lymphohistiocytosis (HLH) is characterized by ectopic EBV infection in CD8+ T cells and their clonal proliferation. We demonstrate that EBV-infected cells can be detected as highly activated CD5-/dim CD8+ T cells that often react with a specific TCR Vβ monoclonal antibody in patients with EBV-HLH, who exhibit characteristic profiles of serum cytokines. Although down-regulation of CD5 in CD8+ T cells is not specific for EBV-HLH, the combined approach of our flow cytometric analysis including TCR Vβ repertoire and cytokine profile analysis may be useful in diagnosis and follow-up of patients with EBV-HLH.
