The optimal approach to relatively recent onset type 2 diabetes patients is still unknown. We speculated that the use of short-acting insulin analogs might be of particular benefit in this context. Patients and Methods. To explore this possibility, we compared the effect on β- and α-cell function of transient intensive insulin therapy using lispro versus human regular insulin in a total of 21 type 2 diabetic patients who were randomly assigned to 14-days intensive insulin therapy consisting of bedtime NPH insulin plus three injections of mealtime lispro (n=11) or regular insulin (n=10). The dosages of both types of insulin were adjusted to attain preprandial glucose levels of <6.1 mmol/l within 1 week with similar rates of glucose decline. An oral glucose tolerance test (OGTT) was performed at day 0 (baseline), 7, and 14; plasma glucose, serum insulin, and plasma glucagon responses over 0-120 minutes were measured, and calculated as the area under the curve (AUC). Results. Lispro led to a significant reduction in glucose-AUC and also an increase in insulin-AUC versus regular insulin on day 7. Glucagon secretion following OGTT was well suppressed with lispro on day 14 compared to regular insulin. Conclusion. Two-week intensive insulin therapy with lispro appeared to be more effective than that with regular insulin in type 2 diabetes in attaining both more rapid β-cell rest and greater suppression of glucagon. These changes may provide significant long-term benefits.","subitem_description_type":"Abstract"}]},"item_4_description_5":{"attribute_name":"提供者所属","attribute_value_mlt":[{"subitem_description":"金沢大学大学院医学系研究科 ","subitem_description_type":"Other"}]},"item_4_publisher_17":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"日本内科学会"}]},"item_4_relation_12":{"attribute_name":"DOI","attribute_value_mlt":[{"subitem_relation_type":"isIdenticalTo","subitem_relation_type_id":{"subitem_relation_type_id_text":"10.2169/internalmedicine.43.779","subitem_relation_type_select":"DOI"}}]},"item_4_rights_23":{"attribute_name":"権利","attribute_value_mlt":[{"subitem_rights":"学会"}]},"item_4_source_id_11":{"attribute_name":"NCID","attribute_value_mlt":[{"subitem_source_identifier":"AA1132885X","subitem_source_identifier_type":"NCID"}]},"item_4_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"0918-2918","subitem_source_identifier_type":"ISSN"}]},"item_4_version_type_25":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_970fb48d4fbd8a85","subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"Murase, Yuko"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"Yagi, Kunimasa"}],"nameIdentifiers":[{},{},{},{}]},{"creatorNames":[{"creatorName":"Sugihara, Masako"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"Chujo, Daisuke"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"Otsuji, Michiko"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"Muramoto, Hiroaki"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"Mabuchi, Hiroshi"}],"nameIdentifiers":[{},{},{}]}]},"item_files":{"attribute_name":"ファイル情報","attribute_type":"file","attribute_value_mlt":[{"accessrole":"open_date","date":[{"dateType":"Available","dateValue":"2017-10-03"}],"displaytype":"detail","filename":"ME-PR-MABUCHI-H-P779.pdf","filesize":[{"value":"91.9 kB"}],"format":"application/pdf","licensetype":"license_note","mimetype":"application/pdf","url":{"label":"ME-PR-MABUCHI-H-P779.pdf","url":"https://kanazawa-u.repo.nii.ac.jp/record/12941/files/ME-PR-MABUCHI-H-P779.pdf"},"version_id":"69e77262-0e12-4695-90cf-c57ab48b580e"}]},"item_language":{"attribute_name":"言語","attribute_value_mlt":[{"subitem_language":"eng"}]},"item_resource_type":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"resourcetype":"journal article","resourceuri":"http://purl.org/coar/resource_type/c_6501"}]},"item_title":"Lispro is superior to regular insulin in transient intensive insulin therapy in type 2 diabetes","item_titles":{"attribute_name":"タイトル","attribute_value_mlt":[{"subitem_title":"Lispro is superior to regular insulin in transient intensive insulin therapy in type 2 diabetes"}]},"item_type_id":"4","owner":"3","path":["1134"],"pubdate":{"attribute_name":"公開日","attribute_value":"2017-10-03"},"publish_date":"2017-10-03","publish_status":"0","recid":"12941","relation_version_is_last":true,"title":["Lispro is superior to regular insulin in transient intensive insulin therapy in type 2 diabetes"],"weko_creator_id":"3","weko_shared_id":-1},"updated":"2023-07-28T01:12:30.995405+00:00"}
