Eighty-eight patients (mean age:61 years) who underwent lung resection without severe cardiopulmonary or other complications were evaluated pre-and postoperatively. We assessed the patients postoperatively after a mean period of 17 days, when they had recovered sufficiently to be able to walk on a level floor at an ordinary pace without dyspnea (Grade 2 by Hugh-Jones). After resection, vital capacity was significantly reduced, but minute ventilation was not reduced indicating ventilative compensation by ventilation rate for loss of tidal volume. However, oxygen consumption (VO_2) and exercise load at the AT were reduced even though heart rate (HR) did not change after resection. On the other hand, postoperative VO_2 at rest was the same as before resection despite of an increase in HR. Thus, the early effect of lung resection on pulmonary function was compensated for by activated ventilation, however, it appears that postoperative reduction in exerdse load at the AT might be mainly due to deteriorations of ventilation equivalent and cardiac output.
