{"created":"2023-07-27T06:28:59.811909+00:00","id":13631,"links":{},"metadata":{"_buckets":{"deposit":"f0db0cbe-60fd-4c68-a7ee-36a4b8227064"},"_deposit":{"created_by":3,"id":"13631","owners":[3],"pid":{"revision_id":0,"type":"depid","value":"13631"},"status":"published"},"_oai":{"id":"oai:kanazawa-u.repo.nii.ac.jp:00013631","sets":["1132:1133:1134"]},"author_link":["22694","22700","22699","22701","22702","480"],"item_4_biblio_info_8":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2008-07-10","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"7","bibliographicPageEnd":"614","bibliographicPageStart":"607","bibliographicVolumeNumber":"112","bibliographic_titles":[{"bibliographic_title":"日本眼科學会雜誌 = Journal of Japanese Ophthalmological Society"}]}]},"item_4_description_21":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"背景：本邦では, 脈絡膜悪性黒色腫に対する眼球温存を目的とした眼内腫瘍切除の予後に関する報告は少ない. 以前, 我々は後極部脈絡膜悪性黒色腫に赤外線レーザー光凝固を併用し, 経硝子体的切除を行った1例について報告した. 初回治療後腫瘍が再発し眼球摘出を行ったので, その後の長期経過も含めて報告する. 症例：47歳男性, 右眼黄斑耳側の脈絡膜腫瘍が約1年の経過で増大し, 脈絡膜悪性黒色腫と判断した. 患者が眼球温存を強く希望したため, 赤外線レーザー光凝固を施行した. 3か月で腫瘍はかえって増大したので眼球温存を図るため経硝子体的に眼内切除を行った. 眼内切除後約4年で腫瘍の眼球内再発がみられたため, 眼球摘出を行った. 病理組織像は, 後極部に限局する紡錘型の悪性黒色腫で, 強膜を貫く浸潤や視神経浸潤は認めなかった. 眼球摘出後約5年で眼窩内再発や遠隔転移は認めていない. 結論：本症例では結果的に眼球を温存できなかったが, 今後, 脈絡膜悪性黒色腫に対する眼球温存療法の手技として眼内切除を考慮するうえで参考となる症例と考える. Background：In Japan, there are few reports of the prognosis of the endoresection of malignant choroidal melanoma. We previously reported this case of transvitreal endoresection of a posterior choroidal malignant melanoma. The tumor recurred and enucleation was carried out. Here we report the subsequent long-term clinical course. Case：A choroidal tumor temporal to the macula was noted in the right eye of a 47-year-old male patient. The tumor size increased within the 1 year of observation and was suspected to be a malignant choroidal melanoma. Since the patient strongly wished to preserve his eyeball, he underwent infrared diode laser photocoagulation, resulting in further progress of the tumor. Three months later a transvitreal piecemeal endoresection was carried out. Four years after endoresection, the tumor recurred, and then the eyeball was enucleated. A pathological examination identified a spindle-cell type malignant melanoma located in the posterior part of the eyeball.
