{"created":"2023-07-27T06:29:08.364910+00:00","id":13833,"links":{},"metadata":{"_buckets":{"deposit":"7f24fa39-c3c2-467b-9059-af707bdc2626"},"_deposit":{"created_by":3,"id":"13833","owners":[3],"pid":{"revision_id":0,"type":"depid","value":"13833"},"status":"published"},"_oai":{"id":"oai:kanazawa-u.repo.nii.ac.jp:00013833","sets":["1132:1133:1134"]},"author_link":["23302","1150","23303","1003","23304","223","22120"],"item_4_biblio_info_8":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2013-01-01","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"2","bibliographicPageEnd":"108","bibliographicPageStart":"102","bibliographicVolumeNumber":"11","bibliographic_titles":[{"bibliographic_title":"形態・機能 = Structure and Function"}]}]},"item_4_description_21":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"我々は先の研究より、上前腸骨棘を通る水平線と大転子中央上縁を通る垂線との交点を新殿筋注点と定め、これがクラークの点と同様に安全な筋注部位であることを明らかにした。本研究では、神経と血管への刺入のより正確な観察を行うために、クラークの点と新殿筋注点にゲル1 mlを注入する手法と、適切な刺入の深さを調べるために超音波診断装置を用いて検討を行った。59~100歳の29御遺体の右側29側において、両点における神経や血管への刺入数は、以前の我々の研究とほぼ同数であり、新殿筋注点の安全性が再度確認された。注入したゲルは、小殿筋と中殿筋の間に固まっていたことから、血管損傷を起こした場合の大きな血腫形成の危険性や、この部位を走行する上殿神経や血管へ影響を及ぼす可能性が示唆された。脂肪と筋の厚さから、クラークの点と新殿筋注点における安全な刺入の深さは皮膚に対して直角に3 cmほど刺入するのが適切と考えられる。しかしながら、より安全に殿部筋肉内注射を実施するには超音波診断装置を用い、脂肪の厚さや血管走行を確認するアセスメントが必要であると考えられる。 In our previous study, we identified a novel intramuscular injection point at the intersection of the horizontal line from the anterior superior iliac spine and the perpendicular line from the middle portion of the trochanter major with the extended hip joint, and clarified that this point is as safe as the point of Clark for intramuscular injection. In the present study, we used 29 right buttocks of cadavers aged 59-100. We injected 1 mL gel into both the point of Clark and our novel intramuscular injection point to examine the relationship of the two points with nerves and blood vessels by investigating whether they were present in the gel. Ultrasound system was used to measure the thickness of the subcutaneous tissue and the gluteal muscle at both points. The incidence of accidental insertion to nerves or blood vessels at the two sites was almost the same as we experienced in our previous study. This indicates that our novel intramuscular injection point provides an appropriate site for intramuscular injection to the gluteal muscle. The injected gel made a mass between the gluteus medius and minimus muscles, suggesting the possibility that hematomas could be formed with negative effects on nerves and blood vessels. Since the thickness of subcutaneous tissue ranged from 1.2 to 2.6 cm at both points, we consider that a depth of 3 cm at a right angle to the skin is appropriate for both the point of Clark and our novel intramuscular injection point.
