| 内容記述 | 本稿の課題は、戦時国民健康保険制度の性格を、主として岐阜県小鷹利村の国民健康保険代行組合の事業報告をもとにあきらかにすることである。戦時国民健康保険制度については、「健民健兵」政策としての評価が通説となっていた。そうした議論では、国民健康保険制度自体の機能と、国策として進められた保健事業とを峻別することなく、当該時期の医療・保健政策の総てを戦時期の特異性として捉えているようである。また、従来の戦時国民健康保険制度史研究では、国民健康保険法の制定過程を検討することで、国民健康保険制度の性格をあきらかにしようとする研究が主流を占め、国民健康保険制度の具体的な機能を評価することについての関心が稀薄であったといえる。これに対し、本稿では、直接には一国民健康保険組合の事業収支の分析から、戦時国民健康保険制度の具体的効果を確認すること、国民健康保険組合の厚生省による「厚生運動」「健民運動」への関与を確認することの二点に重点をおいた。結論として、国民健康保険組合の事業収支を検討した結果、療養給付の支払いが継続して行われ、医療費負担を軽減することで、村民の医療保健要求に応える側面があった。他方、「厚生運動」「健民運動」に協力して進められた国民健康保険組合の事業内容からみれば、戦時体制を構築する側面があったといえる。以上のことから、戦時国民健康保険制度の二側面を確認した。 This article aims to clarify the characteristics of the National Health Insurance System under the wartime regime in Japan. According to the accepted view on this topic, this System was constituted solely as part of constructing wartime regime. In my opinion, this view is a bit one-sided. If we do analysis of the health insurance association(HIA) in each region, we can find that HIA had two features. Firstly, HIA had a strong function as medical insurance which was eagerly desired from most of villagers. I confirmed this function through the concrete analysis of balance sheet made by Kotakari village HIA , Gifu prefecture and found that the burden of medical costs of villagers were greatly reduced. Secondly, so far as the public health services under the wartime were concerned, relationship between HIA and Ministry of Health and Welfare was obviously strong. Therefore, HIA offered this service along the construction of wartime regime. As a conclusion, HIA not only functioned as part of constructing wartime regime but also met the demand of medical care made by most of villagers. In this sense, HIA provided the basis for postwar development of National Health Insurance System. | |||||||
