{"created":"2023-07-27T07:00:57.048933+00:00","id":61064,"links":{},"metadata":{"_buckets":{"deposit":"b9a03e8d-9dec-45a7-866d-f4e7b3cb28c7"},"_deposit":{"created_by":18,"id":"61064","owners":[18],"pid":{"revision_id":0,"type":"depid","value":"61064"},"status":"published"},"_oai":{"id":"oai:kanazawa-u.repo.nii.ac.jp:00061064","sets":["2812:2813:2841"]},"author_link":["22064","20717"],"item_9_biblio_info_8":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2016-03-23","bibliographicIssueDateType":"Issued"},"bibliographicPageStart":"3p.","bibliographicVolumeNumber":"1990 – 1992","bibliographic_titles":[{"bibliographic_title":"平成4(1992)年度 科学研究費補助金 一般研究(B) 研究成果報告書概要"},{"bibliographic_title":"1992 Fiscal Year Final Research Report Summary","bibliographic_titleLang":"en"}]}]},"item_9_creator_33":{"attribute_name":"著者別表示","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{}],"nameIdentifiers":[{},{}]}]},"item_9_description_21":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"1.噴霧吸入によるサーファクタント投与実験:前年度までの研究でサーファクタントの分散液を2ml/kgあて気道内に注入すると、成人型呼吸窮迫症候群(ARDS)の症状が好転することを確認した。そこで、今年度は、噴霧吸入法による投与の効果を検討した。\n(1)肺洗淨ラット:肺洗淨によりサーファクタントを洗い出し、100%酸素による人工呼吸下でも、動脈血酸素分圧(PaO_2)が100mmHg以下のラットを作成した。このラットに、補充療法用サーファクタント(金沢大学で作成したSurfactant CK)を1時間噴霧吸入させたところ、PaO_2が350mmHg(mean)以上に上昇し、肺コンプライアンが24%増加した(P<0.05)。\n(2)エンドトキシン投与ラット:大腸菌由来のエンドトキシンを50mg/kgあて、ラットの気道に注入しARDSを発症させた。このラットに、Surfactant CKを噴霧吸入させると、PaO_2は106mmHg(mean)から300mmHg以上になった(P<0.05)。また、コンプライアンスや胸部レントゲン所見も有意に好転した。\n以上の結果から、噴霧吸入でサーファクタントを投与しても、治療効果が得られると結論された。噴霧吸入法は、注入法に比べて、効果が若干劣るものの、安全性の面で勝れていると判定された。\n2.肺サーファクタントの脂質構成に関する検討(合成製剤の開発):ディパルミトイールレシチン(DPL)、不飽和レシチン(u-L)、ホスファテイジールグリセロール(PG)の3者を種々に組合せ、それに肺サーファクタントのアポ蛋白(SP-BとSP-C)を加えた。これらの試料を、自己の肺サーファクタントが欠如しているウサギ末熟胎仔に投与して換気量を測定した。その結果、u-Lを含む試料を投与した動物の換気量は、含まない場合の約1.5倍であった(P<0.05)。u-Lは、肺サーファクタントの活性を増強する重要な因子であると判定された。","subitem_description_type":"Abstract"},{"subitem_description":"Pathological process of adult respiratory distress syndrome (ARDS) is largely unknown with 50-70% mortality.\n(1) Investigation on the pathological process: An exogenous surfactant was mixed with lung edema fluid sampled from ARDS patients, and administered to the lungs of immature newborn rabbits. When the edema fluid protein-to-surfactant ratio increased from 5.6 to 11.2, the dynamic lung compliance of the animals decreased from 1.08 (mean) to 0.43 ml/cmH_2O/kg (P < 0.05). We conclude that a cause of respiratory failure in ARDS is inactivation of pulmonary surfactant by lung edema that appears during the course.\n(2) Treatment of ARDS by surfactant supplement: Respiratory failure induced by administration of E. Coli endotoxin in rats was largely reversed by supplementation of an exogenous surfactant (100 mg/kg); PaO_2 improved from 106 mmHg (mean) to above 400 mmHg (P < 0.05), and diffuse infiltrates in the chest radiogram disappeared. We conclude that surfactant supplementation can reverse the respiratory failure in ARDS.\n(3) Artificial surfactant for the supplement therapy: Dipalmitoyl lecithin, unsaturated lecithin (u-L), phosphatidylglycerol and the hydrophobic proteins of porcine surfactant were mixed with various ratios. Dynamic lung compliance of the immature newborn rabbits receiving the mixture consists of all the components was 1.24 ml/cmH_2O/kg (mean), but the value receiving the mixture without u-L was 0.79 ml/cmH_2O/kg (P < 0.05).
