Takashi"}],"nameIdentifiers":[{"nameIdentifier":"26064","nameIdentifierScheme":"WEKO"},{"nameIdentifier":"50019618","nameIdentifierScheme":"e-Rad","nameIdentifierURI":"https://kaken.nii.ac.jp/ja/search/?qm=50019618"}]}]},"item_9_description_21":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"電気生理学的検査に基づいた心室性頻拍の直接手術は、不整脈の根治が可能であるため臨床的意義は極めて大きい。本研究では正確な心表面マッピングに頻拍の部位診断法の完成とともに、成功率が高く心筋に対する侵襲が少ない術式の完成をめざした。\n心室性頻拍の心表面マッピング法として15対の近接双極電極を有するマット形電極で電位を採取し、コンピューターで自動作図する方法を考案した。この装置を17例の臨床例に応用した。17例全例で頻拍中の最早期興奮部位の決定が可能であり、内4例(右室異形成2例、心筋炎1例、ファロー四微症1例)では頻拍中遅延電位が記録され頻拍がマクロリエントリーの回路を旋回して生ずることが推定された。この心表面マッピング所見に基づき頻拍の最早期興奮部位、リエントリー回路に直接手術を加え良好な結果が得られた。本装置が十分臨床に応用可能であることが明らかとなり、マット電極内の双極電極の数を25対、30対と増加した装置を開発、臨床に応用し、より詳細なマッピングが可能となった。さらに一心拍で心表面全体からのマッピングを終了できる心表面96点から電位を採取する心表面ネット電極を考案し迅速なマッピングが可能となった。\n心筋に対し侵襲の少ない術式として、より限局した手術が可能なレーザー波、マイクロ波を用いた手術を成犬の心表面からの照射を行い検討した。この結果レーザー波では高エネルギーのため手術時間を短縮できること、術中断層エコーで心筋の照射範囲を同定しつつできること、右心室であれば心表面から十分心内膜の照射を行いえることが確認された。\nまたマイクロ波は凝固止血能に優れていた。しかし復温で伝導路の機能を回復させることができる冷凍凝固に比較し調節性に乏しい。","subitem_description_type":"Abstract"}]},"item_9_description_22":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"研究課題/領域番号:63480318,
