| 内容記述 | 目的:異なる大きさの耳側角膜切開創による白内障手術において惹起角膜乱視変化量の違いを検討した. 対象および方法:対象は無縫合耳側角膜切開白内障手術症例で, 極小切開創1.6mm群が4例5眼, 極小切開創2.2mm群が23例33眼, 小切開創3.0mm群が21例26眼である. 角膜乱視を術前, 術後1週, 1か月および3か月の時点で測定し, 手術による惹起角膜乱視変化量をCravy法による軸乱視量で検討した. 結果:惹起角膜乱視変化量の平均値±標準偏差は術後1週では1.6mm群で0.00±0.69D, 2.2mm群で-0.05±0.61D, 3.0mm群で0.44±0.57Dであり, 術後3か月では1.6mm群で-0.05±0.08D, 2.2mm群で-0.04±0.47D, 3.0mm群で0.47±0.59Dであった. 術後いずれの時点においても2.2mm群では3.0mm群より惹起角膜乱視変化量は有意に小さかった. Purpose:To compare surgically induced astigmatism among 3 sizes of temporal corneal incisions in cataract surgery. Patients and Methods:A retrospective review was carried out of case records of 48 patients(64 eyes) who underwent cataract surgery. The length of the corneal incision was 1.6mm in five eyes, 2.2mm in 33 eyes, and 3.0mm in 26 eyes. Astigmatism was measured by keratometry readings before and 1 week, 1 month, and 3 months after surgery. Surgically induced astigmatism was calculated by vector analyses using the Cravy method. Results:One week postoperatively, the mean surgically induced astigmatism was 0.00±0.69 diopter (D) in the 1.6mm group, -0.05±0.61D in the 2.2mm group and 0.44±0.57D in the 3.0mm group. Three months after surgery, the mean surgically induced astigmatism was -0.05±0.08D in the 1.6mm group, -0.04±0.47D in the 2.2mm group and 0.47±0.59D in the 3.0mm group. The surgically induced astigmatism was significantly less in the 2.2mm group than in the 3.0mm group. Conclusion:The amount of induced astigmatism was significantly less in temporal corneal microincision cataract surgery than in small incision surgery.
