| 内容記述 | 介護支援専門員の在宅認知症者のケアプランの特徴を明らかにし、基礎資格の強みを活かしたケアプランについて検討した。介護支援専門員1,104名にケアプランに関する質問紙調査を実施した。基礎資格に関わらず半数以上が法律機関との連携の難しさを感じていた。6割以上が対象者の受診拒否等に困難を感じており、困難感には法律機関等との連携不足が関連していた。介護福祉士、看護師等の基礎資格を有する介護支援専門員5名と連携を強化する戦略を検討した。 | |||||

| 内容記述 | This study examined the characteristics of care plans for dementia patients living at home by care managers and devised care plans making the most of basic qualifications. A questionnaire survey on care plans was conducted, involving 1,104 care managers. More than 50% of all respondents perceived difficulty in collaborating with legal institutions, regardless of their basic qualifications. Furthermore, more than 60% perceived difficulty due to patients’ refusal to receive medical consultation, and insufficient collaboration with legal institutions was associated with such a sense of difficulty. Based on these findings, strategies to promote collaboration were devised with 5 care managers who had basic qualifications such as care worker or nurse. | |||||
