| 内容記述 | 【目的】本研究は、介護予防事業におけるフットケアの実施状況と課題を明らかにすること を目的とする。 【方法】インターネット上でフットケアを実施している情報が得られた 7 府県の全地域包括 支援センターである。882 施設に対して郵送による自記式質問紙調査を行った。フットケア の実施の実態と課題について単純集計を行い、実施機関別に検討した。 【結果】275 センターから回答があった（回収率 31.2％）。回答のあった施設の内訳は、直営 型地域包括支援センター（以下直営型）53 センター（19.3％）、委託型地域包括支援センター（以下委託型）219 センター（79.9％）であった。フットケアを実施した経験があったのは、59 センター（21.5％）であった。フットケアの位置づけは、転倒防止が 36 センター（61.0％）と最も多かった。方法は、知識と演習の組み合わせが 52 センター（88.1％）であり、演習内容は足指体操が 40 センター（67.8％）、足のマッサージ 39 センター（66.1％）の順に多かった。直営型は委託型に比べ、月 1 回以上実施している割合が有意に多かった（P<0.001）。フットケアを行っていないセンターにおいて必要な体制は、フットケアの指導ができる専門職のサポートが 136 センター（63.0％）と最も多かった。【考察】フットケアを事業として実施したことのある地域包括支援センターは、回答のあったセンターのうち約 2 割であり、実際はさらに少ない割合であると考えられた。現状ではフットケアを実施するスタッフをセンター内で全てを確保することは難しい。地域の中で専門職と連携し、専門職がフットケア事業をサポート出来るような体制作りが必要であると示唆が得られた。 |

| 内容記述 | This study was performed to clarify the implementation status and issues associated with foot care in preventive long-term care. We searched the Internet for community comprehensive support centers where foot care information was being provided to elderly people, and found such centers in seven prefectures. A survey of 882 facilities was conducted by sending self-administered questionnaires by postal mail. We compiled simple statistics on the current conditions and issues associated with foot care implementation and analyzed data according to community comprehensive support center type. We received responses from 275 centers （31.2%）. The facilities were classified as follows: directly operated city or town community comprehensive support centers （direct-managed type）, n = 53 （19.3%）, and outsourced community comprehensive support centers （entrusted type）, n = 219 （79.9%）. Fifty-nine centers had experience with foot care （21.5%）. The most frequent objective for foot care was to prevent falls （n = 36 centers, 61.0%）. Fifty-two centers （88.1%） used both advice and practical foot care exercises. These exercises consisted of foot care and toe movement （n = 40 centers, 67.8%） and foot massage （n = 39 centers, 66.1%）. A significantly higher percentage of directly managed type centers offered foot care more than once a month （P < 0.001）. Of the centers that did not provide foot care, 136 centers responded the requirement for a professional support system for foot care guidance as the reason （63.0%）. The community comprehensive support centers that had implemented foot care accounted for about 20% of the centers that responded, which we consider to be an extremely low proportion. It is difficult to secure all of the necessary staff to implement foot care from only community comprehensive support centers. Thus, it is necessary to cooperate with professionals in communities and to establish a framework in which support can be provided by foot care professionals for foot care projects. |
