| 内容記述 | 膵癌の手術でも、①直接浸潤の範囲と②所属リンパ流域をen blocに切除すべきである。 ①後方組織への浸潤RPに対して腎筋膜前葉は防護策の役割を果たしていた。膵頭部癌では、術前CTで「癌巣が広くIVCに接して且つ一部が突出している」場合に、腎筋膜前葉を越えてIVCに浸潤していた。 ②膵頭部、特に腹側膵領域の所属リンパ流域をmeso-pancreatoduodenumと名付けたが、IPDAを幹とし、発生の腸回転のためSMAの5時方向付近から膵鈎部へと巻くように存在するため、SMAの背側を中心として切除すべきであり、従来言われていたSMA右半周の郭清では取り残しが生じる。 |

| 内容記述 | Resection of lymphatic basin and clearance of direct invasion are required for curative resection of pancreatic carcinoma. The anterior renal fascia acts as a barrier against retropancreatic invasion. The principle underlying our surgical strategy for resectable pancreatic head carcinoma is total excision of the lymphatic basin of the pancreatic head, which is termed meso-pancreatoduodenum (meso-pd). Our meso-pd concept refers to the mesentery of the pancreatic head and the duodenum, which is a firm and well-vascularized perineural lymphatic layer located dorsal to the pancreas that reaches behind the mesenteric vessels. The meso-pd is fan-shaped and its trunk is the inferior pancreatoduodenal artery, which is a tributary of the SMA. Therefore, a total meso-pd resection was performed with respect to the PLX surrounding the SMA and including the anterior renal fascia. | |||||
